Under data protection law, you have the right to access the personal information that the NHS holds about you. This is known as a Subject Access Request (SAR).
You can request access to:
- your GP medical records
- hospital or clinic records
- test results, referral letters, care plans and notes
- information held by the ICB relating to services we commission
You do not need to pay for a copy of your health records. We may ask for identification to confirm your identity before releasing any information.
Requests should be made to the organisation that holds your records. For example:
- your GP practice for primary care records
- your local NHS trust for hospital records
- the ICB for information we hold about commissioning decisions
Some information may be withheld if it includes details about other people or if releasing it could cause harm. We will explain this if it applies.
